Download the forms here, complete the intake process, and then bring your forms, vaccination records, and pictures when you drop your dog off.
In Person
Note, we cannot accept dogs over 25lb and over 18″ or those who will jump a 30″ high gate. If you are unsure, give us a call.
Book Your Service
No reservation is accepted until you receive a confirmation email or text.
If you do not receive a confirmation before 7pm, then we did not receive your text, voicemail, or email. Please, call or email agian.
Intake - Daycare/Boarding
- A signed General waiver form,
- Intake Form,
- A copy of your dog’s current vacines. A ‘tag’ will not due. We need to keep the vacines on record.
- Pictures of all people authorized to pick up the dog. We will only release dogs to people we have a picture of.
Intake - Grooming
Sign the Grooming waiver. If you are coming in with your dog you need to sign the second grooming waiver. We also need a copy of your dog’s vacinations.

This is needed for boarding and daycare. The more detail you add, the better we can care for your dog. If there isn't enough room on the front, add extra instructions on the back.

General Waiver
This is needed for Boarding and Daycare. Please read the whole thing, and fill in every section, including the photo release.

Note: If your dog is matted then we may have no option but to shave them down.
If your dog's nails are so long that the quick will scrape on the ground and open up after you leave, we will refuse the visit.
Accompaniment Waiver
You only need this if you are going to accompany your dog into the grooming room.

If your dog is aging, or ill, please sign this waiver and write out the full details for us.
Step 3: Book Your Appointment
Complete Intake Process
Download and fill in the forms you need.

Submit Vaccination Records
Your dog’s vaccination records must be submitted and reviewed before we can take your dog into our care. You can attach them to your intake papers.
No dog can come on the property without rabies vaccines

Submit Intake Package
You can bring the intake package when you drop your dog off.
We will confirm all appointments.
You must book at least 24 hours in advanced for daycare and boarding, and 5 business days in advanced for grooming.
